  1. Body mass index was calculated based on the height and weight at intensive care unit (ICU) admission. Cardiac assist devices included intra-aortic balloon pumping, extra-corporeal membrane oxygenation, and ventricular assist devices. Fluid balance was defined as an increase or decrease in body weight from ICU admission to enteral feeding tube placement. Hematology included patients with blood disorders, such as leukemia, myelodysplastic syndrome, and malignant lymphoma. Post-cardiac surgery included patients admitted to the ICU for postoperative management of cardiovascular surgery. Respiratory disease included patients with acute respiratory distress syndrome, pneumonia, and acute exacerbation of chronic obstructive pulmonary disease or pulmonary fibrosis. Position of the stomach was defined as the position of the greater curvature of the stomach caudal or cephalad to L1–L2, as estimated by abdominal radiography. Weight was defined as body weight before ICU admission. All values are expressed as number (percentage) or median (interquartile range). SOFA Sequential Organ Failure Assessment
